Create a HTML form in Form Editor for what you want then go to Company>Setup> CRM Campaign Schedule and create an event that uses that form you created, save it and process, voila. 

If you have the picture as a file you can. If you're wanting to use a picture you have in a PDF file that you like but don't have the actual picture file itself you need to get the actual picture file OR use the snipping tool in windows *search "snipping tool" in your run bar* and get your picture that way to save then insert into your new sitelink form.
