Why is Sitelink adding an unauthorized lien charge to a tenants account?

We had an incident where Sitelink added an unauthorized lien fee to a tenants account the other day (we had already done that the day before). We credited the account, but we don't have any idea how or why this happened.
Has anyone else encountered anything like this?
