Bathroom Etiquette and how to enforce it

We have a customer who made quite the disgusting mess in the customer bathroom last week, and we realize IT Happens! We have now noted that he did it again this morning and we recalled that he must have been the one that did it a few weeks ago. He is a grouchy guy that I don't spend too much time talking to, but now..........I feel like I need to say something. This is such a delicate situation. Just wondering how you all might handle this. Thanks in advance!

If you don't address it it will continue to happen. We have a customer that refused to clean up the mess he left. We gave him 2 chances, 2 warnings. Now he is not allowed to use the onsite restroom.
The door is locked when he's onsite, with a note saying "To access, request key from office." He doesn't bother asking.
